If you live in the practice area and would like to become a patient of the practice, please ask our reception for a Pre Registration Application Form, You should complete this fully and leave at reception.

When asked by reception please bring your medical card to the surgery, if you have lost your Medical Card please ask for a HS200 form, or if you have never had a UK Medical Card ask for a HSCR1 form.

All newly registered patients will be asked to complete a simple questionnaire and may be invited for a “new patient” check up / assessment. This enables us to meet you and assess your medical needs.

Please note you can choose to attend any of the doctors in the partnership not necessarily the one you are registered with.

We do encourage you to consult one doctor for chronic problems as this ensures continuity of care.

Practice Area

The practice area covers a five-mile radius from the surgery, east of the River Lagan to Purdysburn Village, Carryduff, Moneyreagh, Gransha, Gilnahirk, Tullycarnet, Dundonald and Sydenham. There is a regular bus service within 100 yards of the surgery.
